Liability Waiver & Release

I am voluntarily participating in activities offered by Good Vibes Wellness LLC ("Good Vibes Wellness"), which may include paddleboard yoga, outdoor fitness classes, land-based yoga, hiking, and other wellness and outdoor activities (collectively, the "Activities").

I understand that the Activities involve inherent risks of physical injury, property damage, and even death. These risks include, but are not limited to:

• Water-based risks: drowning, near-drowning, hypothermia, water inhalation, currents, waves, weather changes, collisions with the paddleboard or other watercraft, falls from the paddleboard, entanglement, and aquatic wildlife.
• Physical activity risks: muscle strains, sprains, fractures, joint injuries, cardiovascular events, dehydration, heat-related illness, and aggravation of pre-existing conditions.
• Environmental risks: sun exposure, insect bites, allergic reactions, lightning, wind, cold, slippery surfaces, uneven terrain, and wildlife encounters.
• Equipment risks: failure, malfunction, or improper use of paddleboards, paddles, leashes, life jackets, mats, or other equipment.
• Other risks: risks I cannot foresee, including risks caused by the negligence of Good Vibes Wellness, its instructors, or other participants.

Colorado law limits what a parent can waive on behalf of a minor. By completing this section, you give permission for the minor to participate and release any claims you personally may bring as the parent (such as loss of consortium or medical expenses).

In exchange for being allowed to participate, I, on behalf of myself, my heirs, family, executors, administrators, and assigns, release, waive, and discharge Good Vibes Wellness LLC, its owners, instructors, employees, contractors, volunteers, agents, and venue partners (collectively, "Released Parties") from any and all claims, demands, actions, or liability arising from my participation in the Activities, including claims based on the negligence of any Released Party, to the fullest extent permitted by Colorado law.

I understand this means I cannot sue the Released Parties for injuries, damages, or losses I suffer, even if those injuries are caused by their carelessness or negligence.

I agree to defend, indemnify, and hold harmless the Released Parties from any claims brought by me or on my behalf, and from any costs (including reasonable attorney's fees) the Released Parties incur defending such claims.

Good Vibes Wellness may photograph and record participants during activities and use those images for marketing, social media, websites, and printed materials.
